Now Hiring: Hospice RN (GA)

THA Group’s Island Hospice has an immediate opening for a Registered Nurse (RN) to care for hospice patients in Georgia.

Responsibilities: Serves as a professional member of the hospice interdisciplinary team to coordinate skilled nursing are to hospice patients. Responsible for assessing patient and family needs and providing quality patient care in order to promote positive outcomes that are consistent with the established plan of care.

Essential Functions:

  • Performs admission visits, post-hospital evaluations, death visits, discharges and other visits as assigned.
  • Completes required documentation in a timely manner and uses the nursing process to monitor patient status.
  • Provides skilled nursing care and instruction to patients and/or caregivers as directed by the plan of care.
  • Submits nursing notes, visit logs, and physician orders in a timely manner; Initiates verbal reports on patients as needed in order to promote effective communication and ensure continuity of care.
  • Assesses patients’ physical and emotional status; reports changes in patients’ status to the physician and care manager in a timely manner; initiates additional physician orders as needed.
  • Educates patients, families, and caregivers on end of life processes and disease progression.
  • Ensures that all patients and /or their caregivers understand their rights and responsibilities.
  • Collaborates with other members of the clinical team to implement, update, and evaluate the plan of care for assigned patients from admission through discharge.

Qualifications:

  • Registered Nurse (RN) with current state licensure; B.S.N. preferred
  • One to two years experience in nursing; specific experience in hospice strongly preferred
  • CPR certified
  • Effective communication skills
  • Reliable transportation with proof of insurance, current driver’s license and good driving record

Now Hiring: Spiritual Counselor

 

POSITION SUMMARY:

THA Group’s Island Hospice has an immediate opening for a Spiritual Counselor to provide spiritual support to patient/family in their belief system, in a manner consistent with Hospice policy and procedure. The Spiritual Counselor is in an integral position on the Interdisciplinary Team and provides support to team members.

ESSENTIAL FUNCTIONS:

  1. Assesses the hospice patient/family to identify spiritual needs and develops a Plan of Care in accordance with the findings.
  2. Participates in the development and implementation of the Plan of Care to ensure quality and continuity of care.
  3. Works with staff, clergy, and community groups to enhance their sensitivity to the spiritual concerns of patients/families experiencing terminal illness and loss.
  4. Performs occasional liturgical assignments such as the Annual Service of Remembrance, and conducts or makes arrangements for funeral or memorial service when asked by families.
  5. Makes contact with clergy or appropriate representatives of patients/families as indicated.
  6. Demonstrates commitment, professional growth and competency.
  7. Promotes Agency philosophy, mission statement and administrative policies to ensure quality of care.

SKILLS, KNOWLEDGE, ABILITIES, TRAITS:

  • Exhibition of good communication skills, attention to guest relations and focus on customers.
  • Contribution to a positive work climate and to the overall team effort of the department in a manner, which exemplifies the values, and philosophy of the organization.
  • Adherence to department and organizational policies and procedures.
  • Demonstration of environmental awareness in work area and organization.

EDUCATION AND/OR WORK EXPERIENCE:

  1. Graduate of accredited seminary or school of theology or appropriate certification in pastoral ministry.
  2. Two (2) years experience in personal counseling.
  3. One (1) year experience in group counseling preferred.
  4. Excellent human relation and oral/written communication skills; time management and organization skills, must be able to manage multiple tasks simultaneously.
  5. Reliable transportation with proof of liability insurance, valid driver’s license and a good driving record.

Travel Required:  No overnight; local travel only
