Now Hiring: Hospice Patient Care Coordinator / Team Lead (Savannah, GA)

THA Group’s Island Hospice has an immediate opening for a full time Hospice Patient Care Coordinator/Team Lead. This role coordinates all aspects of patient care transition for Island Hospice to ensure that patients/families understand the benefits of hospice and that admissions occur in a timely, efficient, and patient-centered manner. Additionally, this role serves as Team Lead in coordinating care for hospice patients and promoting positive outcomes that are consistent with the established plan of care.


  1. Provides supervision, oversight and patient care coordination for Hospice Care Managers
    • Participates in the training and continued education of hospice team members as needed
    • Collaborates with Performance Excellence to ensure efficient and effective orientation for new employees
    • Ensures field evaluations and skills check offs are completed in a timely manner for all direct reports
    • Provides leadership to team members through coaching, counseling, performance evaluations, and other performance management techniques
    • Manages team activities (PTO, patient assignments, etc.) to facilitate consistent patient care
  2. Serves as a resource for hospice team members in areas of expertise (medication management, hospice protocol, nursing skills, etc.)
  3. Collaborates with Hospice AVP to identify and implement enhancements to improve patient care and satisfaction
  4. Monitors the financial components of Hospice operations to ensure optimum efficiency without compromising patient care
  5. Assists with the creation and implementation of Island Hospice policies and processes
  6. Conducts or coordinates quality audits, chart reviews, and other activities as indicated to ensure continued high quality in patient care
  7. Develops constructive working relationships with hospital and referring physicians to facilitate referrals and ease of transition to Hospice
  8. Interacts with THA Group Business Development professionals to coordinate patient/family meetings and educate on Hospice and palliative services
  9. Coordinates the process of patient referral and admission
    • Reviews referral information
    • Verifies that Intake has received appropriate financial authorization
    • Contacts Hospice administrative support to schedule admission visit
    • Assures that documentation to support hospice admission has been received (Certificate of Terminal Illness, Face to Face)
    • Arranges for any necessary DME, IV supply delivery, medications
  10. Ensures smooth handoff to Hospice Care Manager
    • Verifies patient’s actual discharge date for hospitalized patients
    • Obtains final discharge orders and discharge diagnosis
    • Notifies patient of who their Care Manager will be and when they can expect a visit
    • Tells the patient/family what they can expect from the first visit
    • Discusses medications
    • Reinforces “Call Us First” as first point of contact for concerns or emergencies
  11. Documents summary of patient needs
    • Disciplines required
    • Arrangements made for DME, IV supplies, meds, etc.
    • Red flags found during visit
  12. Attends IDT meetings to ensure continued communication regarding Hospice patients and their families
  13. Works with Medical Directors to ensure communication of patient referrals and admissions as needed
  14. Completes all documentation in a timely manner and in compliance with company policy and regulatory guidelines.


  1. Registered Nurse with current Georgia and South Carolina state licensure
  2. Advanced degree or commensurate experience strongly preferred
  3. CHPN certification strongly preferred
  4. Minimum 2 years hospice experience in nursing; experience as lead or supervisory clinician preferred
  5. CPR certified
  6. Reliable transportation with proof of insurance, current driver’s license and good driving record
  7. Demonstrated ability to manage complex, multi-faceted situations that can be dynamic in nature
  8. Thorough understanding of patient-centered hospice care and the regulatory and financial components of hospice case management
  9. Ability to coordinate and work through others to accomplish desired outcomes
  10. Excellent customer service and interpersonal skills
  11. Excellent verbal and written communication skills
  12. Demonstrated ability to prioritize and organize to meet multiple deadlines
